A series of direct photographic measurements of corneal reflex positions was taken to reconcile discrepant interpretations of the Hirschberg test, and a value of 21 prism diopters per millimeter was obtained for the conversion factor relating ocular rotations to reflex displacement, valid over a range of 200 prism D. A series of direct photographic measurements of corneal reflex …

A value of 21 prism diopters per millimeter was obtained for the conversion factor relating ocular rotations to reflex displacement, valid over a range of 200 prism D. Quantitative interpretation of routine clinical photographs of strabismic patients can thus be easily obtained by including a millimeter scale in each picture.

Calibration of Hirschberg test photographs under clinical conditions. DeRespinis PA(1), Naidu E, Brodie SE. Author information: (1)Manhattan Eye, Ear and Throat Hospital, New York, NY 10021. The feasibility of determining the angle of ocular deviation from clinical photographs was assessed. Corneal reflex photographs were obtained from 30

Calibration of the Hirschberg test in human infants

    https://pubmed.ncbi.nlm.nih.gov/8113004/
Purpose: This study was designed to determine empirically whether the adult value of the Hirschberg ratio (approximately 22 prism diopters/mm) is a suitable value to use for all ages. Because many structures in the eye are growing during the 1st year, it is possible that there could be some change in the Hirschberg ratio during this period.

The Hirschberg test is, of course, most important clinically in the evaluation of patients who either lack central fixation or lack the ability to cooperate with prism-cover testing. To compare the results of the photographic measurement with prism-cover testing, these patients were deliberately excluded from our study.

Hirschberg test, uses of Hirschberg corneal light reflex test

    https://healthjade.net/hirschberg-test/
The Hirschberg test is considered normal when the corneal light reflexes are slightly decentered nasally (about 5º, due to angle kappa). If the light reflexes fall asymmetrically in the pupils, strabismus may be present. In the case of a hypertropia, the light reflex of the deviated eye is located below the light reflex of the fixing eye.
